What you''ll get to do:
The Care Coordinator position is critical to the Medica Care System. The position is responsible for assessing the medical and social needs of the Medica Care System products' members and ensuring they receive all the necessary care and services they require. Duties include the following:
Act as the primary contact for member needs
Perform assessments in the community, at facilities or via telephone
Create an appropriate individualized care plan and coordinate the delivery of the approved medical and social services
Interface with physicians, providers and county financial entities to discuss the care plan and provide updates on member progress as a result of implementation of the plan of care interventions
Key Accountabilities:
Assessment and service coordination of Medica Care System community and institutional members
Coordination of medical, social services, mental health and chemical health on behalf of members
Develops care plans for members and communicates progress with physicians, member, family and providers
Active participant in the Medica Care System Team
Qualifications:
Bachelor''s degree in Nursing preferred
Nursing: 2 years home or community based experience or PHN with at least one year of home and community based services

Experience:
Experience in home and community based services/case management
Experience working with those who have a disability and with seniors
Comfortable working with diverse, low income populations
Excellent communication skills including appropriately simplifying medical language into a format understandable to non-medical trained members
Ability to shift gears quickly and prioritize tasks to meet needs of members and the Medica Care System
Certifications/Licensure:
Current licensure as a RN in the state of MN
MnCHOICES Certified Assessor Training (MNCAT) preferred
